How medical information about you may be used and disclosed
This notice also explains how you can obtain access to your information. Please review it carefully. It applies to health information maintained by Beyond Fertility, Inc. and the workforce members and service arrangements identified by the clinic as covered by this notice.
You have rights over your health information
Get a copy of your medical record
You may ask to inspect or receive an electronic or paper copy of your medical record and other health information we maintain about you. We generally provide a copy or summary within the time required by law and may charge a reasonable, cost-based fee.
Ask us to correct your record
You may ask us to correct information you believe is incorrect or incomplete. We may deny a request in certain circumstances and will explain the reason in writing.
Request confidential communications
You may ask us to contact you in a specific way or at a different location. We will accommodate reasonable requests.
Ask us to limit use or disclosure
You may request restrictions on certain uses or disclosures. We are not always required to agree. If you pay in full out of pocket and ask us not to disclose information about that service to your health plan for payment or operations, we will agree unless disclosure is required by law.
Get a list of disclosures
You may request an accounting of certain disclosures made during the period allowed by law. It does not include every disclosure, such as most disclosures for treatment, payment or healthcare operations.
Get this notice and choose a representative
You may request a paper copy at any time. If someone is legally authorized to act for you, we will recognize that authority after verification as required.
Tell us what you prefer
Where applicable, you may tell us how to share information with family, friends or others involved in your care and how to respond in a disaster-relief situation. If you cannot communicate a preference, we may act in your best interest and disclose information needed to reduce a serious and imminent threat to health or safety.
We will obtain written authorization for uses and disclosures that require it, including most uses of psychotherapy notes, most marketing uses and the sale of protected health information. You may revoke an authorization in writing except to the extent we have already relied on it.
How we may use and disclose information
Treatment
We may use and share your health information to provide, coordinate or manage your care and to communicate with professionals involved in your treatment.

We may use and share information to bill and obtain payment from health plans or other responsible parties.
Healthcare operations
We may use and share information to operate the clinic, improve care, train staff, manage risk and contact you when necessary.
Other purposes allowed or required by law
Subject to legal conditions and limits, we may use or disclose information for public-health and safety activities, health oversight, organ and tissue donation, workers' compensation, law enforcement, legal proceedings, research, medical examiners, certain government functions and compliance with other laws.
Special federal or California protections may apply to certain information, including reproductive health, substance-use treatment, mental health, genetic testing, HIV status and minor-consent services. We will follow the more protective law when it applies.
