Pre-launch review required. This is a website-ready template. Before publishing, Beyond Fertility must confirm its secure form provider, hosting, analytics, cookies, vendors, retention periods and official privacy contact. Protected health information is addressed separately in our HIPAA Notice.
01 / Scope

About this policy

This Privacy Policy explains how Beyond Fertility, Inc. ("Beyond Fertility," "we," "us" or "our") collects, uses and discloses information through this website and related online interactions that link to this policy.

This policy does not replace our Notice of Privacy Practices. If information is protected health information under HIPAA, that notice governs how we may use and disclose it.

02 / Collection

Information we collect

Information you provide

  • Identifiers and contact details, including name, email address, telephone number and mailing address.
  • Appointment-request information, including the service you are interested in and any message you choose to provide.
  • Communications you send to us and information provided when you contact the clinic.

Information collected automatically

When configured for production, the site and its service providers may collect device and usage information such as IP address, browser type, operating system, pages visited, referring page, approximate location and timestamps. This list must be updated to match the technologies enabled at launch.

Please do not include highly sensitive medical details in a general website form unless the form is clearly identified as a secure clinical communication channel.

03 / Use

How we use information

  • Respond to questions and appointment requests.
  • Coordinate requested services and communicate about next steps.
  • Operate, secure, troubleshoot and improve the website.
  • Maintain records, prevent misuse and comply with legal obligations.
  • Send marketing communications only where permitted and subject to your choices.

Submitting the website form does not create a physician-patient relationship and does not guarantee an appointment.

04 / Disclosure

How we disclose information

We may disclose information to service providers that support website hosting, secure forms, communications, scheduling, security and professional services; to authorities or other parties when required by law or necessary to protect rights and safety; and in connection with a merger, financing, acquisition or transfer of business assets.

Where HIPAA applies, vendors that receive protected health information must be handled in accordance with HIPAA, including a business associate agreement when required.

05 / Technology

Cookies and tracking

The production website may use essential cookies needed for security and functionality. Any analytics, advertising pixels, session-replay tools, chat widgets or embedded scheduling services must be listed here before launch, together with available controls.

Because this is a healthcare website, advertising trackers should not be enabled on appointment, registration or patient-facing pages until privacy and HIPAA review is complete. Browser privacy signals and cookie controls will be honored where required by law.

06 / Choices

Your choices

You may ask to access, correct or delete certain information, unsubscribe from marketing emails using the link in the message, or change browser controls for cookies. Some requests may be limited by medical-record, legal, security or operational requirements.

For rights concerning medical records and other protected health information, review the HIPAA Notice.

07 / California

California privacy information

California residents may have rights under applicable privacy laws. Depending on the law and any available exemption, those rights may include requesting access to, correction of or deletion of personal information and information about collection and disclosure practices.

If the California Consumer Privacy Act applies to Beyond Fertility, this section must be updated before launch with the required categories, request methods, metrics and opt-out or limitation mechanisms. We do not discriminate against anyone for exercising an applicable privacy right.

08 / Stewardship

Security and retention

We use administrative, technical and physical safeguards designed to protect information. No website or transmission method can be guaranteed completely secure. We retain information for as long as reasonably necessary for the purposes described here, including clinical, legal, accounting and security needs. Final production retention periods must be confirmed by the clinic.

Children

This website is not directed to children under 13 and we do not knowingly collect personal information from them through the general website. A parent or legal guardian should contact us regarding care for a minor.

Changes

We may update this policy as our practices or legal requirements change. The date at the top shows when it was last revised.
